Submit the completed form via email to info@gotosca.org or by regular mail and send check for annual dues of $35 made out to TOSCA and address to Doug Mills, 5061 Berean Lane, Irvine, CA 92603. Annual dues must be received within 30 days of the receipt of the membership form.
